How to Navigate Our Grounds
Lakefield Cemetery organizes its grounds into rows of graves that run parallel to Buckhorn Road. We refer to each of these rows as a Range. Depending on the section, a Range consists of either a single or double row of graves, each bordered by a walkway. To help you navigate, we further divide double rows into East and West designations.
Understanding Lot Numbers
Originally, we divided each Range into 16-grave Lots and 8-grave half-lots. You can identify these by their specific designations: letters D through A represent the southern sections, while numbers 1 through 28 continue toward the north. Furthermore, we divide each Lot into quarters for precise location.
Modern Sections
Please note that our most recent sections vary from this traditional layout. In these areas, we have moved to individually numbered graves, which are clearly marked on our map for your convenience.

Interment Permission & Ownership Transfer Forms
Permission for Interment
Current Rights Holders or legal heirs may use this form to authorize interments and memorialization. While this document grants permission for use, it does not transfer ownership of the Interment Rights.
Timing and Usage
You may submit this form as a pre-authorization to be kept on file, or at the “time of need” for an immediate service.

Within 3 years of death: An executor may complete this form on behalf of a deceased Rights Holder.

After 3 years: All surviving children or grandchildren must sign the document in accordance with the Succession Law Reform Act, 1990.
Filing and Fees
Once filed, the cemetery will only permit the individuals named on this form to be interred or memorialized. However, these permissions are flexible; you may revoke or update them at any time by providing written notice to the Cemetery Office.
Requirements
Please note that you must file all permissions and pay the associated fees—as outlined in our Cemetery Price List—before any interment occurs or any memorial is installed.

Transfer of Ownership
Current Rights Holders or legal heirs use this form to permanently transfer ownership of interment rights to a new party. Unlike a permission form, this document legally changes the registered owner of record at the Cemetery.
Authorized Signatories

Within 3 years of death: The executor may complete this transfer on behalf of the deceased Rights Holder.

After 3 years: In accordance with the Succession Law Reform Act, 1990, all surviving children or grandchildren must sign the transfer.
Rights and Responsibilities
Once filed, the new Interment Rights Holder(s) gains the exclusive authority to direct all future interments and memorialization for the grave, lot, or niche.
Fees and Completion
To finalize the change, you must submit the completed document and pay the transfer fee specified in our Cemetery Price List.

Permission & Transfer Checklist
Before contacting our office or submitting your forms, please ensure you have the following documents ready. Having these prepared will help us process your request more efficiently.
- The Original Certificate: For ownership transfers, we require the original Interment Rights Certificate. If this document is lost, please contact us for a duplicate copy (A fee may apply).

If acting as an Executor: A copy of the Will (specifically the pages naming the executor and the signature page).

If the Rights Holder is deceased (over 3 years): A list of all surviving children or grandchildren to comply with the Succession Law Reform Act.

Identification: A clear copy of government-issued photo ID for all individuals signing the forms.

Completed Form: The relevant Permission or Transfer form downloaded from the section above.

Payment: Check the Cemetery Price List for the current filing or transfer fees. We accept Cash, Cheque, Debit, E-transfer, Visa or Mastercard.

Official Approval
To give our families peace of mind, the Bereavement Authority of Ontario (BAO) has reviewed and approved our By-Laws. This ensures that our local governance aligns with provincial laws and consumer protection standards.
