Permission Forms

Ontario Law provides that there shall be no interment without the consent of the owner of the burial rights. If the original owner has died without transferring the remaining plots to individuals, the correct interpretation of “owner” is all those who have an interest in the burial rights. The Lakefield Cemetery will accept authorization for interment using the following forms, depending on the need. It is the executor’s responsibility to assure that authorization is obtained from all those entitled before the burial can take place.

Permission for Interment

Permission for Interment by Owner

This form may be used by the current living Interment Rights Holder registered at the Lakefield Cemetery office or the Power of Attorney of the current living Interment Rights Holder.

If the Interment Rights Holder is deceased, the executor of the estate may use this form up to a period of three years from the date of death.

If the Interment Rights Holder is deceased, in lieu of the executor, all the immediate children must give permission for the interment.

Permission for Interment by Children

This form is to be used to authorize an interment in a grave where the Interment Rights Holder is deceased and the lot has not been designated to another in the will.

All the immediate children are required to give permission for the interment to take place.

Transfer of Intermet Rights

Transfer Endorsement by Owner

This form is to be used by the current living Interment Rights Holder, registered at the Lakefield Cemetery office, to transfer unused graves to another person.

Permission to Transfer Interment Rights by Children

This form is to be used to authorize the transfer of unused interment rights in a grave where the Interment Rights Holder is deceased and the lot has not been designated to another in the will.

All the immediate children are required to give permission for the transfer to take place.
